ご意見を聞かせていただきたいのですが
皆さんがある病院の患者だとして、、、
皆さんの個人的医療データーを病院側に提供することに承諾しますか？（ＯＰＴ－ＩＮ）
それとも　拒否しますか（ＯＰＴ－ＯＵＴ）
承諾された場合は　病院のデーターにインプットされ、
　これからの医療現場で他の患者さんのトリートメントの参考として使われます。
承諾または拒否する理由もきかせていた…
