腰椎穿刺 禁忌

初めまして ふと授業で疑問に思ったのですが・・・・
腰椎穿刺の禁忌で頭蓋内圧亢進のある患者とありましたが
小児の髄膜炎では腰椎穿刺をして検査しますと習いました
しかし髄膜炎でも頭蓋内圧亢進症状があります
なぜリスクが高いのをわかってて腰椎穿刺するのですか？
脳ヘルニアの危険性もあるのに・・・
もし知っていたら教えてください
