再発防止策(カーテンの閉め忘れ)について

母(介護職)に代わり質問させていただきます。
Ｄｒと看護師とで入院患者のバルーンの交換の
ため閉めていたベット周りのカーテンを開け、「
閉めるのを忘れたため」、始末書を書くことにな
りました。
始末書には「事故発生状況」と「再発防止策」と
自分の氏名・押印をしなければなりません。
Ａ4用紙の上からどの順番で書けばいいでしょう
か？(始末書には決まった書式はありません…
