私は現在、うつ病と診断され９月３日より休職しています。
社会保険は昨年の５月に加入していますので１年以上経過しています。
診断書は既に出ており、傷病手当の１回目申請書を本人が書く項目は記入し
現在は病院に提出しており、１０月１日に取りに行き、会社に提出する
予定になっています。
