慢性疼痛疾患管理料の算定について

整形外科で働いている医療事務初心者です。
慢性疼痛疾患管理料について教えてください。
慢性疼痛疾患管理料には変形性膝関節症、筋筋膜性腰痛症以外にどのような病名がありますか？？
「診療所である保険医療機関において慢性疼痛に関わる疾患を主病とする外来患者に対して療養上の指導を行った場合に月1回に限り算定できる」とありますが、注射や投薬のみで受診されている患者様には算定できるのでしょうか？？
