医療レセプト記入方

今通信で医療事務の資格取得を目指しているのですが終了試験で疑問点が２つあるのでどなたか教えて下さい
病院(200床)患者は２歳
処方についてで同じ外用薬を計３回処方
そのうち２回は量が同じなのでまとめて記入出来ますがあと１回は量が違うのでこの場合のレセプト記入の際は別々に書くのでしょうか？
あと１つは注射(iV)について
計２回実施
１回目は手技料が算定出来ないので薬剤料を算定

レセプト減点について

薬剤を持参された患者様で、注射の手技料だけを算定し、コメントとしてとしたのですが
不必要と判断され減点されました。この様な場合どう算定すれば、良いでしょうか？
