傷病手当金請求書について

病気で仕事を休みました。会社から傷病手当金請求書の書類を貰いました。
通院した、病院に記入をして貰いましたが、文書作成料として一通につき3000円請求されました。
給料締日がまたがった為二通依頼したので、6000円でした。でも傷病手当金請求書は、レセプト請求出来る書類だと思います。病院が間違ったのでしょうか？
それとも、一般の診断書や死亡診断書は各医療機関で金額が自由に設…
